Your First Visit
Payment PoliciesSurgical Instructions
 
Your initial appointment will consist of a consultation to review your diagnosis and treatment options. An estimated fee will be provided. Occasionally, surgery can be done the same day as the consultation. However, a complex medical history or treatment plan necessitating intravenous anesthesia will require lead time for scheduling.

If you wish to undergo surgery the same day as your consultation, please be sure to have nothing to eat or drink for 8 hours prior to your appointment. It will be necessary for a responsible adult to accompany you to the appointment, drive you home and stay with you for the remainder of the day. Please review the pre-surgical instructions for further details.

We greatly appreciate your assistance in providing the following information at the time of your consultation:

  • Your completed registration forms
  • Your surgical referral slip from your general dentist, along with any recent X-rays taken (see additional X-ray instructions below)
  • A list of medications you are presently using
  • If you have dental insurance, bring your identification card. This will save time and allow us to help you process any claims.

X-rays

If your referring dentist has taken X-rays, please request that they forward them to our office. In the event that your consultation is scheduled within a week of your appointment with your referring dentist, please pick up your X-rays personally and bring them with you to the appointment.

Please note: All patients under the age of 18 must be accompanied by a parent or legal guardian for the consultation visit.
